About this test
CT TEMPORAL BONE is a specialized computed tomography examination that produces high-resolution images of the bones and air spaces surrounding the ears. The temporal bones are located on both sides of the skull and contain the external ear canal, middle ear, delicate hearing bones, inner-ear structures, facial nerve canal and mastoid air cells. Because these structures are extremely small and closely arranged, a dedicated thin-slice CT protocol is used to display their anatomy in detail.
The examination may be performed for one ear or both ears, depending on the referring doctor's prescription and the patient's symptoms. Images are commonly obtained in thin axial sections and reconstructed in coronal, sagittal or specialized oblique planes. These views help radiologists and ENT specialists assess bone anatomy, air spaces and the relationship between different structures.
A routine CT Temporal Bone is often performed without intravenous contrast when the primary concern involves bone anatomy, chronic ear disease, hearing structures or trauma. Contrast may be added in selected circumstances involving suspected complications, infection, inflammation, a mass or another soft-tissue concern. The exact protocol should be confirmed by the radiologist according to the clinical indication.

Why Doctors Recommend This Test
Doctors may recommend CT Temporal Bone for patients with persistent or recurrent ear infections, hearing loss, ear discharge, ear pain or suspected chronic middle-ear disease. The scan can show whether the middle-ear cavity or mastoid air cells are filled with abnormal soft tissue or fluid and whether nearby bones have been affected.
One important indication is the evaluation of suspected cholesteatoma. A cholesteatoma is an abnormal collection of skin cells within the middle ear or mastoid that can erode nearby bone. CT may demonstrate the location and extent of abnormal tissue and associated bony changes. However, CT cannot always distinguish cholesteatoma from other soft tissue. Clinical examination and, in selected cases, specialized MRI may be needed.
The examination may be used to assess conductive hearing loss when the doctor suspects an abnormality of the ossicles or other middle-ear structures. The ossicles are three tiny bones called the malleus, incus and stapes. CT can demonstrate their alignment and certain forms of damage, fixation or developmental abnormality. Audiometry and other hearing tests remain essential because imaging does not measure hearing function directly.
CT Temporal Bone may also be requested after significant head trauma. It can identify fractures involving the temporal bone and demonstrate possible involvement of the ear canal, middle ear, ossicles, inner-ear capsule or facial nerve canal. Trauma patients with hearing loss, facial weakness, severe dizziness, bleeding from the ear or clear fluid leakage require urgent specialist assessment.
Other indications include suspected otosclerosis, congenital ear abnormalities, persistent dizziness associated with a suspected bony condition, facial nerve canal assessment, surgical planning and evaluation before cochlear implantation. MRI may be preferred when the main concern involves nerves, the brain, soft-tissue tumours or the internal auditory canals. CT and MRI answer different clinical questions and may sometimes be complementary.
Preparation Before Test
Most non-contrast CT Temporal Bone examinations require little preparation. Patients can generally eat, drink and take their regular medicines normally unless the diagnostic centre or referring doctor provides different instructions. Do not stop antibiotics, ear medicines or other prescribed treatment unless advised by the treating doctor.
Bring the doctor's prescription, hearing-test reports and previous imaging. Earlier CT scans, MRI scans, audiograms, tympanometry results, operative notes and ENT records can help the radiologist interpret the findings and assess changes over time.
Remove earrings, spectacles, hearing aids, hairpins, removable dental appliances and other metallic objects near the head when instructed. Metal can create streak artifacts that obscure the small structures of the temporal bone. Inform the technologist about cochlear implants, hearing implants, previous ear surgery or other implanted devices.
Intravenous contrast is not routinely required for every temporal-bone CT. If contrast is prescribed, fasting and a recent kidney-function test may be necessary. Inform the team about kidney disease, diabetes, asthma, thyroid conditions, significant allergies, current medicines and any previous reaction to iodinated contrast material.
Pregnant patients or anyone who may be pregnant must inform the referring doctor and CT staff before the examination. CT uses ionizing radiation, so the healthcare team will assess whether the examination is necessary and whether another imaging method could answer the clinical question. Children require appropriately adjusted scanning protocols.
During the scan, the patient lies on a motorized table with the head positioned carefully. Cushions or supports may be used to reduce movement. Remaining still is especially important because the structures being assessed are very small and movement can significantly reduce image quality. The scan itself is usually completed quickly and is painless.
Normal Reporting Time
The image-acquisition portion of CT Temporal Bone is generally completed within a few minutes. Registration, preparation and careful positioning increase the total appointment time. Additional processing is required to produce high-resolution images and specialized reconstructed views.
A radiologist reviews the thin cross-sectional images and multiplanar reconstructions before preparing the report. Many routine reports may be available within 24 to 48 hours. Complex congenital abnormalities, postoperative cases, implant assessments or examinations requiring comparison with previous imaging may take longer. Who Should Take This Test?
CT Temporal Bone may be appropriate for patients with chronic or recurrent ear disease, persistent ear discharge, unexplained hearing loss, suspected cholesteatoma, possible ossicular abnormality, ear-related trauma or a congenital ear condition. It may also be recommended before selected ear operations or cochlear implantation.
The examination should be performed when recommended by an ENT specialist, audiologist working with a physician, surgeon, neurologist or another qualified doctor. It is not a routine screening test for every episode of ear pain. Many common ear conditions can be diagnosed through direct examination, hearing tests or other investigations without CT.
Children and patients requiring repeated imaging need special consideration because CT involves radiation. The expected diagnostic benefit should justify the exposure, and the imaging team should use an appropriate protocol. Pregnant patients also require individualized assessment before scanning.
Patients with sudden hearing loss, new facial weakness, severe vertigo, clear fluid leaking from the ear, significant head trauma, high fever with swelling behind the ear or rapidly worsening symptoms should seek urgent medical assessment rather than waiting for a routine appointment.

A CT scanner uses a rotating X-ray source and electronic detectors to collect information from multiple directions around the head. A computer converts these measurements into very thin cross-sectional images. High-resolution reconstruction algorithms emphasize fine bony details within the temporal bone.
The external ear canal carries sound toward the eardrum. Behind the eardrum lies the middle ear, which contains the malleus, incus and stapes. These tiny ossicles transmit sound vibrations toward the inner ear. CT can display their shape and alignment, although direct examination and hearing tests are needed to determine how well they function.
The inner ear includes the cochlea, vestibule and semicircular canals. The cochlea is associated with hearing, while the vestibular structures contribute to balance. CT is useful for evaluating the bony anatomy of these structures. MRI provides more detailed assessment of many nerves and soft tissues and may be recommended when the clinical concern extends beyond bone.
The mastoid portion of the temporal bone contains multiple air cells connected to the middle ear. In chronic ear disease, these cells may become opacified or poorly aerated. CT may also demonstrate bony erosion, sclerosis or postoperative changes. The imaging findings must be correlated with otoscopy, symptoms and microbiological results when infection is suspected.
In suspected cholesteatoma, CT may show soft tissue in the middle ear or mastoid with erosion of the ossicles, scutum or other bone. Similar soft-tissue appearances can occur with fluid, inflammation or granulation tissue. Specialized diffusion-weighted MRI may be recommended in selected cases, particularly when recurrent or residual cholesteatoma is suspected after surgery.
For otosclerosis, CT may show abnormal bone near the stapes or within the bony capsule surrounding the inner ear. A normal CT does not exclude every case, and the diagnosis also depends on clinical findings and audiometry. Treatment decisions should be made by an ENT specialist.
In temporal-bone trauma, fractures may be described according to the structures involved rather than only by their direction. CT can show injury to the ear canal, ossicular chain, mastoid, inner-ear capsule, carotid canal or facial nerve canal. A dedicated CT angiographic examination may be required if vascular injury is suspected.
Before cochlear implantation, CT may help demonstrate cochlear shape, mastoid anatomy, facial nerve canal position and congenital variations. MRI may also be required to evaluate the auditory nerves and soft tissues. Preoperative assessment therefore often combines imaging, hearing tests and specialist evaluation.
CT exposes the patient to ionizing radiation. The dose depends on scan coverage, patient size and technical settings. The radiology team selects an appropriate protocol to obtain diagnostically useful images while limiting unnecessary exposure. Providing earlier imaging can help comparison and reduce avoidable repeat scans.
After a routine non-contrast examination, patients can generally resume normal activities immediately. If intravenous contrast is administered, follow the centre's instructions regarding hydration, observation and symptoms that require medical attention. The final report should be reviewed by the referring ENT specialist or doctor, who will correlate the imaging findings with symptoms, ear examination and hearing-test results.
